Epidemiology of agricultural malaria frontier: factors associated with infection and disease in population-based study in rural Brazilian Amazonia.

Abstract

Understanding the epidemiology of malaria in Brazil, to support malarias control, has attracted interest mainly due to its high morbidity in populations continuously exposed to the risk of infection. Between March 2010 and April 2011, four cross-sectional were performed in a farming settlement located in the state of Amazonas, with the recruitment of 395 patients aged between two months and 66 years. Considering the molecular results obtained by real time PCR in 731 blood samples, we obtained a positivity rate of 17.8%, nearly three times greater than that detected by microscopic examination, with the detection of mixed infections only by molecular technique. About two-thirds of diagnosed infections were asymptomatic. The results emphasize the importance of asymptomatic infections, often submicroscopic, to the maintenance of the transmission of parasites and characterize the role of behavioral issues related to the age and cumulative exposure to malaria.
