Abstract

This clinical, prospective, pragmatic, covert, and opportunistic study, which is part of the thematic project FAPESP 2017/12815-0, will assess the interference of training in the confirmability of sedation scales, numerical scale, Short Form of the Glasgow Scale for evaluation of Acute Pain in Dogs and University of Melbourne Scale. Thirty evaluators will be selected, one of them is a researcher student and there are subdivided into the groups "gold standard", "undergraduates", "residents", "postgraduates", "surgery" and "clinical", they will evaluate 80 videos of 20 dogs, consistent with baseline (M1). They will be filmed for 12 hours after adaptation to the environment and preoperatively to avoid interference from fasting, greater pain (M2). In addition, the recording will be between one to eight hours post-extubation, post-analgesia (M3). Consistently, one hour will be filmed after rescue analgesia and 24 hours after extubation (M4). Therefore, ten dogs will be evaluated in the pre-and post-training phases, with two stages for each evaluation, fifteen days apart so it will not have any memory interference. Moreover, in both evaluations, two dogs will be from the control group, four from the orthopedics group, and four from the soft tissue group, with the equivalence of pain scores between stages. It will be considered ± 5% and the repeatability and reproducibility will be evaluated before (pret.t) and after (pos.t) the training of numerical scales by weighted Kappa coefficient and 95% CI. To assess the abbreviated sedation scale and the Glasgow (CMPS-SF) and Melbourne (UMPS) scales, the "two-way" intraclass correction coefficient (ICC) and 95% CI will be applied. To assess the effectiveness of training in the studio. The Feldt test will be applied and the significant difference will be established when the F value is greater than the critical value of ± = 0.01. The evaluators will be considered able to assess sedation and pain in dogs when they reach inter- reliability e 0.80 with respect to the "gold standard" group. To assess the interference of gender and dependent variable scores among the different groups, Wilcoxon (parametric) or Mann-Whitney (non-parametric) tests will be used in order to test the difference between the two assessments of each step. Finally, the responsiveness of the scales will be assessed using the Friedman test between all times and groups, and separate analyzes will be carried out between M1 x M2 to check whether they agree in the identification of behaviors related to sedation and acute pain.
