Characterization of movement avoidance behavior in patients with different shoulder disorders

Abstract

Studies have shown that there is a correlation between fear of pain related to movement (kinesiophobia) and shoulder impairment. Patients with anterior traumatic instability, for example, have a high kinesiophobia score according to the Tampa Scale of Kinesiophobia (TSK) questionnaire. Still, the literature indicates that psychological factors such as fear related to pain or painful activities significantly contribute to the function and intensity of pain perceived in the arm in individuals with adhesive capsulitis. In trauma, in turn, greater pain intensities were found in patients with kinesiophobia behaviors. On the other hand, in individuals with subacromial pain (SPS), pain intensity and fear beliefs significantly contributed to the deficiency at the beginning of the study, however, the literature is still scarce regarding the influence of kinesiophobia in patients with SPS. Thus, the aim of the study is to characterize the avoidance of movement behavior in patients with different shoulder disorders, through the photographic Avoidance Daily Activities Photo Scale (ADAP Shoulder Scale), also investigating whether clinical factors such as clinical diagnosis, time of symptoms, age, sex, level of pain and disability in the shoulder and psychological factors such as resilience, kinesiophobia and self-efficacy are capable of explaining the ADAP Shoulder Scale. Therefore, the study will consist of a sample of 120 patients of both sexes, aged between 18 and 60 years, and diagnosed with anterior traumatic instability, adhesive capsulitis, SPS, and shoulder trauma. The assessment will consist of filling out self-reported scales such as the ADAP Shoulder Scale for movement avoidance, TSK for kinesiophobia, PSEQ for self-efficacy, SPADI for pain and disability, as well as the Brazilian Version of the Connor-Davidson Resilience Scale (RISC-Br). Therefore, the data will be computed and treated statistically. As a hypothesis of the present study, it is expected to find high scores on the ADAP Shoulder Scale in patients with anterior traumatic instability, adhesive capsulitis, SPS and shoulder trauma. Furthermore, it is expected that the ADAP Shoulder Scale will be explained by some of the clinical and psychological factors. (AU)
