Abstract

Oral Mucositis is a clinical condition that frequently affects patients under antineoplasic therapy for head and neck cancer. The disease is correlated with oral mucosa ulcerations and intense pain, odynophagia, increase in the risk for infections, use of antibiotics and period of hospitalization. The correlation among Oral Mucositis, fungemia and the potential of systemic fungal dissemination was recently described. Despite of the impact of this clinical condition on the lifetime and quality of life in cancer patients, there is no consensus on the prophylaxis and therapeutic protocol. Low Temperature Atmospheric Pressure Plasma (LTAPP) has antimicrobial, anti-inflammatory and tissue repair effects. These features suggest that it might be a promising tool in the treatment of Oral Mucositis. The aim of this project is to evaluate the application of LAPTT as adjuvant in the treatment of Oral Mucositis. A total of 120 rats (Rattus norvegicus) with 90 to 100 days of age will be included in the study. Oral Mucositis will be induced by systemic adminstration of cisplatin and 5-fluorouracyl associated with topic application of 50% acetic acid. The animals will be randomly divided into 4 experimental groups (n=30): i) Oral Mucositis; ii) Oral Mucositis+LTAPP; iii) Oral Mucositis infected with C. albicans and iv) Oral Mucositis infected with C. albicans+LTAPP. Treatments with LTAPP will be performed in 2 sessions after the diagnosis of Oral Mucositis. During the experimental period, the lesions will be photographed and their areas will be measured. The local microcirculation will be assessed by images of videomicroscopy analyzed by AVA 4.0 software (sidestream dark field imaging). One, 3 and 5 days after the last treatment, group of 10 animals will be euthanized. Specimens will be submitted to histologic and immunohistochemical analyses (Ki-67 and anti-VEGF), RT PCR (expression of TNF-alpha, IL-1², IL-10 and Nrf2, HO-1, Nqo1) and cell oxidation evaluation. The occurrence of fungal systemic dissemination in group of Mucositis infected with C. albicans will be assessed by fungal isolation from total blood and organs macerated. The results will be analyzed statistically by suitable tests to be selected according to data distribution and the evaluation. The level of significance will be set at 5%. (AU)
