Prevalence of pregnant women with pre-diabetes (HbA1c between 5.7 and 6.4%) in the first trimester of pregnancy

Abstract

Some questions are raised about the screening and diagnosis of diabetes mellitus in pregnancy. These include the use of glycated hemoglobin (HbA1C) routinely in the first trimester of pregnancy to diagnose pre-existing diabetes in the first trimester. To confirm this diagnosis, the HbA1C value is well established (equal to or greater than 6.5%). However, during pregnancy, HbA1c values between 5.7 and 6.4% are not considered, which in the general population is considered as prediabetes and a change in lifestyle is recommended, and there may be a need for association with medications. Objective: (I) to determine the prevalence of pregnant women with overt diabetes, gestational diabetes mellitus (GDM) and pregnant women with HbA1C levels between 5.7-6.4% identified up to 14 weeks of gestation. B Method: observational study, cross section. Will be eligible for the study pregnant women who had dosed fasting glucose and glycated hemoglobin in the first trimester of pregnancy. In the laboratory evaluation of the first trimester of pregnancy, women will be classified into three groups: (I) normal, with glycated hemoglobin (HbA1c) below 6.5% and fasting glucose (GJ) below 92 mg / dL; (II) DMG, with HbA1c below 6.5% and GJ between 92 and 125 mg / dL; and (III) overt diabetes, with HbA1c equal to or greater than 6.5% and / or GJ equal to or greater than 126 mg / dL. Pregnant women considered normal will still be divided into two groups, according to glycated hemoglobin: (Group 1) normal with HbA1c less than 5.7%; and (Group 2) prediabetes with HbA1c equal to or greater than 5.7% and less than 6.5%. From this information it will be estimated the prevalence of pregnant women with overt diabetes, GDM and pregnant women with levels of HbA1C between 5.7-6.4% identified up to 14 weeks of gestation, with their respective confidence intervals of 95%.
