Abstract

Rheumatoid Arthritis (RA) is an autoimmune disease characterized by the involvement of synovial joints, causing pain and swelling. Moreover, clinical manifestations presented by patients with RA may contribute to worsening of physical capacity, quality of life, increasing risk of morbidity and mortality cardiovascular and development of a sedentary lifestyle in these patients. It is known that aerobic exercise programs and strength, performed at moderate to high intensity can improve aerobic capacity, muscle strength, functional capacity, cardiovascular risk factors and body composition in patients with RA. The benefits promoted by physical exercise in patients with RA is undeniable. However, a growing body of evidence suggests that, in addition to compliance with the recommendations of physical activity, individuals should aim to reduce the time spent in sedentary behavior to promote improvements in health. Few studies have shown that breaking up prolonged time spent in sedentary behavior is associated with improvement in cardiovascular risk factors and reduced risk of all-cause mortality. In this regard, it is plausible to assume that interventions aiming to promote reduction of sedentary time and breaking up prolonged sedentary behavior with light physical activity could be beneficial on comorbidities and cardiovascular risk factors in patients with RA. Moreover, to the best of our knowledge, no studies in the literature aimed to determine whether an intervention to promote the reduction of sedentary time and increasing breaking up sedentary behavior is effective in patients with RA. In this context, the aims of this study are: (1) to evaluate the sedentary time, number of breaking up in sedentary time and physical activity level in patients with RA; (2) to investigate the effects of an intervention that promotes breaking up prolonged sedentary time in disease-related parameters, body composition, cardiovascular risk factors, physical function and quality of life in patients with RA. Moreover, to gather knowledge on the mechanisms involved on the breaking up prolonged sedentary behavior in skeletal muscle of these patients. Two studies will be conducted as part of this project. The first is a descriptive cross-sectional study of association (study 1), while the second is a longitudinal, randomized and controlled (study 2). Patients will be recruited at the Rheumatology Unit at the Clinical Hospital of Faculty of Medicine, University of São Paulo (HC-FMUSP). Data collection will be conducted at the Laboratory of Evaluation and Conditioning in Rheumatology. The volunteers will be analyzed for the following parameters: (1) level of (in)physical activity (accelerometry and ActivPAL®); (2) global clinical evaluation (specific questionnaires); (3) anthropometry and body composition (DXA); (4) inflammatory and hematological blood parameters; (5) cardiovascular risk factors (Framingham Survey); (6) aerobic capacity (maximal graded exercise test); (7) functional capacity and fatigue (functional tests and specific questionnaires); (8) quality of life (SF-36); (9) muscle biopsy (study 2).
