Abstract The aims of the present study will be: a) quantify endotoxins and bacterial CFU counts in primary endodontic infections before and after biomechanical preparation (BP) and the use of root canal medication (RCM) in case of multiple-visit treatment; b) to correlate the levels of endotoxins and bacterial CFU counts with the volume of periapical bone destruction (VPBD) determined by Cone Beam Computed Tomography (CBCT) analysis as well as with the development of clinical signs and symptom; c)connect the levels with the volumes of root canals; d)compare the volume of periapical lesions before and after 1 year of endodontic treatment in cases of single-session and multiple session. Thirty teeth with pulp necrosis and apical periodontitis will be selected and submitted to CBCT analysis. After access to pulp cavity, a first root canal sampling will be perform in order to determine the presence of root canal infection. Afterwards, BP will be perform using 2.5% NaOCl followed by the placement of RCM according to the selection group: GI - Calcium Hydroxide [Ca(OH)2] - Ca(OH)2 + Saline Solution (SSL); GII -Calcium Hydroxide paste, (Ultracal®) and GIII - single-visit root canal treatment. A second collect will be made after the BP and then the third after 14-days of RCM. Culture analysis was performed in order to determine antimicrobial activity. The levels of endotoxins were determined by the Limulus assay. The volumes of periapical bone destruction will be accomplished by using CBCT Nemotec® software. In addition, will be sustained a control of endodontic treatment, to compare the volumetric periapical lesions before and after 1 year of treatment protocols.
