Abstract

Grounding: In parallel with the worldwide obesity epidemics, there is an alarming increase in the prevalence of insulin resistance (IR) and metabolic syndrome among children and adolescents, which are disease states of great and growing importance in terms of morbidity and mortality. Therefore, it becomes important to identify practical methods, accurate and applicable in population studies and in clinical practice to predict insulin resistance in adolescents, because early detection provides healthcare professionals the opportunity to adopt prophylactic and therapeutic measurements.Objective: Validate substitutive methods, laboratory and anthropometric, as insulin resistance indicators studied by the hyperglycemic clamp in adolescents with different levels of adiposity and pubertal stages.Methods: Multicentre cross-sectional study of 835 adolescents (10 to 19 years). There will be assessed: pubertal stage, blood pressure, waist and neck circumference (NC), sagittal abdominal diameter (SAD), BMI z-score, fasting glucose, fasting insulin, total cholesterol and fractions, uric acid, liver enzymes and lean mass. The IR will be assessed by the Homeostasis Model Assessment-adiponectin (HOMA-AD) and by the hyperglycemic clamp. The correlation between two variables will be evaluated by partial correlation coefficient adjusted for pubertal stage. The association between the SAD, the NC and HOMA-AD with the clamp will be evaluated by chi-square test. The degree of agreement between the SAD, the NC and HOMA-AD with the clamp will be evaluated by Kappa coefficient. To evaluate the performance of the NC, the SAD and HOMA-AD to identify insulin resistance assessed by the clamp ROC curves will be constructed and the areas under the curves and their 95% CI will be calculated. To determine the cutoff of NC, the SAD and HOMA-AD to identify we will consider the 90th percentile of the healthy subgroup. Significance level p<0.05.Perspectives: The results of this study should contribute to providing diagnostic tools of insulin resistance applicable in population studies and in clinical practice.
