Abstract

The selective IgA deficiency the most common primary immunodeficiency, which is susceptible to respiratory infections, gastrointestinal alterations, allergic symptoms, as well as prone to the development of autoimmune diseases. The homeostasis of the immune system depends on the balance between the innate and adaptive immune responses. The innate response recognizes the molecular patterns associated with microbial, MAMPs by Toll-like receptors (TLR), important sensors to be evaluated functionally, through the production of cytokines. Adaptive response is crucial the balance between the regulator cell subset, such as regulatory T cells, with the pro-inflammatory cells population, named Th17 cells. The aim of this project is to evaluate in selective IgA deficiency the profile of response to the agonists of TLRs and the frequency of regulatory CD4+ T cell as well as the proinflammatory/polyfunctional TCD4+ cells. This will be evaluated in patients with selective IgA deficiency compared to healthy individuals the cytokine production induced by synthetic agonists of TLRs (2-9) in peripheral blood mononuclear cells, the frequency of naïve/effector regulatory cells, CD4+ CD25+ CD127 -CD45RA-FoxP3 +, CD4 + T cells secreting IL-17a and IL-22 as well as IL-2, IL-21, IFN-³ and the determination of IL-17a, TNF-±, IFN-³ and IL-10 serum levels. These data may be indicators of the balance between innate and adaptive response, and the regulatory mechanisms and pro-inflammatory in the pathogenesis of the selective IgA deficiency.Keywords: Selective IgA, Toll-like receptors, regulatory T cells, Th17, Th22.
