Abstract

Vogt-Koyanagi-Harada (VKH) disease is an autoimmune disease that targets melanocyte related proteins of genetically susceptible individuals. Its clinical course is subdivided into four phases: prodromal (flu-like symptoms), uveitis, convalescent and chronic. Pigmentary alterations of the choroid and skin may be observed in the course of either convalescent and/or chronic phases. Some authors have drawn attention to the progression of these alterations in cases with little or none overt inflammation and have correlated this phenomenon with disease severity. Da Silva et al. conducted a prospective transversal study (supported by FAPESP grant 07/57.155-5 and scholarship 07/57.154-9) in which 30 to 60% of patients had poor outcome. Thus, questions related to the efficacy of the initial treatment and also related to the effetiveness of disease and treatment monitorization in the late stage arise. The present longitudinal and prospective study, with a minimum one-year follow-up, aims to: 1. a global analysis of patients in the acute phase treated with methilprednisolone pulsetherapy; 2. a global analysis of late stage patients; and; 3. evaluate the impact of systemic treatment when disease worsening is detected by electrorretinographic exams. Global analysis is defined as disease activity evaluated by clinical exams and indocyanine green angiography, structure (fundoscopy and spectral optical coherence tomography) and function (electrorretinography and fundus autofluorescence). The results expected from this study are: understand the relevance of the analysis of each parameter (clinic, structure, function) in the acute phase and late stage and to monitor disease progression and treatment outcome.
