Abstract

The use of potassium citrate for the treatment of hypocitraturia is routinely used in current clinical practice. However, due to its high cost and side effects, especially with regard to gastrointestinal intolerance, it is necessary to study other effective treatment for hypocitraturia. This study aims to evaluate the effects of administration of sodium bicarbonate on the urinary excretion of citrate in hypocitraturic stone formers. Twenty hypocitraturic stone formers attending the Clinic of Renal Lithiasis of the Federal University of Sao Paulo, will be selected. Patients will be randomized in two groups to receive 30 mEq of sodium bicarbonate three times per day or 10 mEq of potassium citrate three times daily. Each period will last for three days and will be held in the presence of controlled diet. All patients will undergo blood collection and 24-hour urine at baseline and after the use of sodium bicarbonate or potassium citrate for determination of creatinine, sodium, potassium, calcium, phosphorus and serum uric acid and blood gas determinations and urinary creatinine, sodium, potassium, calcium, phosphorus, magnesium, uric acid, citrate, oxalate and pH urinário. If demonstrated the potential effect of sodium bicarbonate to increase citraturia, it can be established as an alternative treatment for hypocitraturia.
