Abstract

Rotator cuff tears are frequent in the population. Among the affected tendons, the subscapularis has received little attention in the orthopedic literature, although such lesions are evident in up to 40 % of arthroscopies Recent studies have reported low sensitivity of magnetic resonance imaging (MRI) in the detection of these tears, which is in contrast to the results of systematic reviews that assessed all rotator cuff tendons. We will perform a prospective study, evaluating the accuracy of the MRI in the detection of the subscapularis tears. The MRI will be compared with the diagnostic gold standard (arthroscopic surgical inspection). We will include 200 shoulders, submitted to arthroscopic treatment due to affections of the rotator cuff. MRI will be performed with a 1.5-T magnet. Two radiologists and one orthopedist will be blindly evaluators. The orthopedist will re-evaluate the images in a random order in a second time. We will analyze the sensitivity, specificity, positive predictive value and negative predictive value, intraobserver and interobserver agreement. The aim of this study is to evaluate the accuracy of preoperative MRI for the identification of subscapularis tears. Our secondary aim is the identification of factors that may be predictive for the presence of subscapularis tears. (AU)
