Uterine transplantation program of Hospital das Clínicas - FMUSP - a pilot project

Abstract

Uterine transplantation is an experimental therapy for absolute uterine infertility. Therefore, it is indicated for cases of uterine malformation without the possibility for surgery correction, early hysterectomy due to neoplasm or hemorrhage and intrauterine adherences (Asherman Syndrome). None study of uterine transplantation has been conducted in our country until this present date.5 patients will be selected from those assisted in gynecological clinic with uterine malformation or who underwent to a hysterectomy due to cervical neoplasm or leiomyomas and desire to conceive a child. Before the transplantation, a performance of the assisted reproduction with gonadotropin for ovarian stimulation, oocytes aspiration and cryoconservation of embryos will be conducted. A compatible deceased donor will be selected for each patient. For the immunosuppression, basiliximab, tacrolimus, azatioprin, micophenolate mofetil and corticosteroids are going to be administered until the child's birth and the doses will be adjusted for each patient, depending on their evolution.Embryos will be located in the transplanted uterus at 6 months after the transplantation. After confirmation of the pregnancy, the patient will be assisted for high risk prenatal cares and monitored in case of rejectio. After the child's birth, the uterus will be removed. (AU)
