Influence of risk factors for chronic periodontitis on the periodontal and systemic immunoinflammatory profile

Abstract

Diabetes mellitus and smoking are recognized risk factors for periodontal diseases. However, there is insufficient evidence in the literature regarding the direct comparison of the local and systemic immunoinflammatory profile related to the chronic periodontitis in smokers and type 2 diabetic subjects. There is also scarce evidence regarding the immunoinflammatory profile in periodontitis patients presenting both conditions (i.e. smoking diabetic subjects). Therefore, the aims of this study will be: 1- to compare the levels of a wide panel of cyto/chemokines, related to inflammation and T helper cell responses, in the gingival crevicular fluid (GCF) and serum of smokers, type 2 diabetic subjects and smoking diabetic subjects with chronic periodontitis, by means of a multiplex bead immunoassay; 2- to compare the levels of 7 bacterial species related to periodontitis in biofilm subgingival samples of smokers, type 2 diabetic subjects and smoking diabetic subjects with chronic periodontitis, by means of a real time PCR; 3- to correlate the cyto/chemokine profile in serum and GCF with the levels of periodontal pathogens present in the subgingival biofilm of smokers, type 2 diabetic subjects and smoking diabetic subjects with chronic periodontitis. One hundred fifty subjects (30/group) will be allocated in one of the following groups: (1) uncontrolled diabetic subjects with chronic periodontitis, (2) smoking subjects with chronic periodontitis, (3) smoking diabetic subjects with chronic periodontitis and (4) non-smoking/non-diabetic subjects with chronic periodontitis (5)non-smoking/non-diabetic subjects without periodontitis. The subjects will be submitted to periodontal clinical examination and gingival crevicular fluid (from healthy and diseased sites) and blood samplings. Gingival crevicular fluid and serum samples will be analyzed for the levels of eighteen cyto/chemokines including GM-CSF, INF-gama, IL-1 beta, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12 (p70), IL-13, IL-17, IL-21, IL-23, MIP-1 alfa,TNF-alfa and TGF-beta, using a multiplex bead immunoassay. Subgingival biofilm will be evaluated by quantitative PCR for the following species: Prevotella intermedia, Porphyromonas gingivalis, Tanerella forsythia, Eubacterium nodatum, Parvimona micra, Fusobacterium nucleatum and Treponema denticola. Statistical analysis will be performed according to data normality by ANOVA or Kruskal Wallis tests. Adjustments will be made for multiple comparisons, as the levels of 18 cyto/chemokines will be evaluated in the same sample. Models of multivariable logistic regression analysis will be undertaken to estimate the association between diabetes, smoking, diabetes + smoking, clinical parameters, the local and systemic levels of the cyto/chemokines and the levels of periodontal pathogens. The level of significance will be set at 5% for all analysis. (AU)
