Abstract

Fetal gastroschisis is an abdominal wall defect associated with extrusion of internal organs through the defect and direct exposure to the amniotic fluid, without peritoneal membrane covering the exteriorized content. The incidence of gastroschisis varies from 2-3 to 4-5 to 10,000 births, however with significantly increase in the last decades. Despite elevated postnatal survival rates, approximately 90%, morbidity is high with prolonged hospital stay, and therefore it is considered one the malformations with the longest period of post repair hospitalization. The etiology of gastroschisis is unknown therefore is considered as due to environmental factors, however the strong association with very young women indicate the need of further research involving environmental, nutritional and genetic factors. Recent studies showed an association between maternal diet quality and fetal gastroschisis. Also maternal imbalance of fatty acids was associated with fetal gastroschisis. However these were retrospective studies based on post natal interviews or on patients records. Therefore there is need for further prospective studies to confirm these findings. This is a prospective case-control study involving 56 pregnant women with fetal gastroschisis, alive fetus and gestational age < 34 weeks. Each gastroschisis mother will have two pregnant women with a normal fetus as a control matched for age, body mass index and gestational age. Food questionnaire will be applied at the study inclusion. Maternal blood will be collected at the study entry and at the delivery time. Umbilical cord blood sample will be collected soon after birth. Fatty acids concentration will be measured by gas chromatography. Nutritional and anthropometric state will be evaluated by body mass index and food questionnaires. It is expected that these results will help to clarify the contribution of nutrition in the etiology of fetal gastroschisis. (AU)
