Clinical evaluation of resin-modified glass-ionomer restorations of cervical lesions predominantly affected by abfraction or erosion

Abstract

The non-carious cervical lesions (NCCL) are an ongoing challenge in operative dentistry. They are characterized by loss of tooth structure regarding physiological and/or pathological wear which can lead to dentin exposure ultimately. The main consequences range from dentin sensitivity to aesthetic and functional discomfort to the patient. Though the NCCL commonly present multi-factorial etiology, they are very often treated without any distinction. The identification of the major etiologic factor, its intensity and morphological aspects can contribute to the prevention and treatment of the NCCL. The aim of this clinical trial is to evaluate the performance of resin-modified glass-ionomer restorations of NCCL predominantly caused by abfraction or erosion. 70 patients will be recruited and distributed into two groups (n = 35): abfraction and erosion previously identified by clinical, photographic and microscopic examinations and clinical history. Restorative procedures will be performed with no cavity preparation by two calibrated operators. Evaluations will be made with a modified USPHS method in the baseline, after 6 months, 1, 3 and 5 years by two calibrated examiners regarding the criteria: retention (main endpoint), marginal adaptation, marginal staining, post-operative sensitivity, adjacent caries, color alteration and anatomic form. The agreement between the examiners will be tested through the Kappa index. Evaluations at different periods will be analyzed with McNemar test, whereas between groups (abfraction X erosion) with Fisher's exact test, both with significance level of 5%. (AU)
