Level of physical activity and fitness and its association with pulmonary function decline in adults in the city of Santos, Brazil

Abstract

Few studies have evaluated extensively the level of physical activity in daily life (PADL) in adults using a combination of motion sensors, questionnaires and physical fitness tests. Moreover, the causal association between physical inactivity/sedentary lifestyle and the development of chronic obstructive pulmonary disease (COPD) requires further elucidation. The objectives of this study will be to evaluate in adults: (1) the prevalence of physical inactivity and its associated factors, and (2) the association between level of physical activity and fitness and clinically significant pulmonary function decline and/or development of COPD over time. At least 624 participants aged 20 to 80 years of the 31 districts of the city of Santos/SP will be evaluated and followed for four years with annual reassessments. Demographic data, clinical examination, biochemical tests, anthropometry, body composition, spirometry, electrocardiogram, physical fitness and PADL (measured by triaxial accelerometry and questionnaire) will be evaluated. Those who do not perform at least 150 minutes/week, 5x/week of moderate physical activity and do not carry at least 10,000 steps/day (insufficiently active) and those who do not perform at least 10 minutes/week of physical activity according to the results of the International Physical Activity Questionnaire (IPAQ) (sedentary) will be considered physically inactive. In cross-sectional study we will assess prevalence of physical inactivity and associated factors using logistic regression. In the four years follow up cohort study, the decline in forced expiratory volume in one second (FEV1) and forced vital capacity (FVC) will be calculated by: final valuemL - initial valuemL/timeyears. The variables of physical activity and fitness that can predict annual decline in pulmonary function will be assessed in multiple linear regression models. The clinically significant decline in pulmonary function, set at reduction in FEV1 > 50 mL/year, and diagnosed cases of COPD over time will be registered. The influence of physical inactivity in the risk of decline in FEV1 > 50 mL/year and/or COPD in this period will be evaluated in the Cox regression model adjusted for main covariates. By identifying the factors associated with physical inactivity and confirm their causal association with the decline in pulmonary function and/or diagnosis of COPD, this study will serve to outline strategies, including exercise, for prevention of COPD whose spending on health resources are extremely high in Brazil.
