Abstract

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by bilateral growth of numerous cysts, which distorts normal kidney architecture. Besides being associated with an increased left ventricular mass, hypertension is also associated with increased renal volume in ADPKD. In ADPKD, the activation of the Renin-Angiotensin-Aldosterone System (RAAS) can occur either by the reduced production of nitric oxide as well as the expansion of bilateral cysts. Recent observational studies have shown that the concentration of 25 (OH) D levels (vitamin D) is inversely related to blood pressure. Studies suggest that the prevalence of hypovitaminosis D is common in the general population, indicating a low intake of vitamin D and low sun exposure. In ADPKD, studies that assess the "status" of vitamin D are scarce. Recently, preliminary data from Gitomer et al revealed that patients with ADPKD 25 (OH) D d 30ng/mL had a higher renal volume when compared to patients with sufficient vitamin D levels, after adjustment for gender, height, hypertension and season. Considering the paucity of studies evaluating serum levels of vitamin D in a population with ADPKD and their association with hypertension and other clinical parameters, nutritional and laboratory parameters related or not with the activity of the renin-angiotensin-aldosterone system, it justifies the need to conduct an observational clinical study.
