Validity and reproducibility of an incremental step test in patients with chronic obstructive pulmonary disease

Abstract

The determination of the functional capacity has been considered an important part of assessment in patients with chronic obstructive pulmonary disease (COPD), especially by using testes which are representatives of daily activities. In this context, the step test seems to have advantage over other functional capacity tests because it is considered a portable test, simple to perform it, and free of costs. The main objective of this study is to analyse the reproducibility of an incremental step test, and secondarily to compare the exercise responses between the stepping and cycling tests. Admission criteria include: clinical diagnosis of COPD (forced expiratory volume in the first second, FEV1 < 70% predicted), clinical stability as indicated by no change in medication dosage in the preceding four weeks, and absence of associated locomotor or neurological conditions. After study enrollment, at the first visit patients will perform the incremental step test (IST), and one hour of rest, they will perform the incremental cardiopulmonary test on cycle. At the second visit, at least 24 hours apart, patients will repeat the incremental step test. Our hypotesis is that the IST will show a similar pattern of exercise responses compared to the incremental cycle ergometer test. Therefore, it could be used as a novel tool to evaluate exercise responses and after interventions with advantage of being a simple test, which requires minimal space to perform it, and it does not depend on patient motivation.
