Influence of spironolactone on cardiac remodeling induced by tobacco smoke


In addition to the well known vascular effects of tobacco, some studies suggest that this exposure could induce ventricular remodeling. Tobacco smoke exposure results in left ventricular enlargement and hypertrophy, systolic and diastolic dysfunction evaluated by in vivo and in vitro techniques. The mechanisms involved in this remodeling process are not completely known, and the role of inflammation and renin-angiotensin-aldosterone system (RAAS) on cardiac remodeling induced by tobacco smoke exposure was not study yet. RAAS blockade with aldosterone blockers, as spironolactone, have been studied in different models of ventricular remodeling. Recently their use is recommended by American Heart Association for patients with systolic dysfunction and heart failure with NYHA class III and IV, and for systolic dysfunction after myocardial infarction. Potential effects of aldosterone blockers are myocardial inflammation and hypertrophy attenuation. Thus our hypothesis is that spironolactone could attenuate remodeling process induced by tobacco smoke exposure, through the reduction of myocardial inflammation and hypertrophy. (AU)
