Arterial stiffness, Genetic Analysis, Cytokines concentration Inflammatory, microparticles and transfer of lipids in Patients with Ischemic Stroke for Hypertensive Emergency

Abstract

Introduction - Hypertensive crisis (HC) can lead to lesions in target organs featuring hypertensive emergency. Among these lesions, there is the ischemic stroke (AIS). Among the possible factors related to HC, stands out arterial stiffness, polymorphisms of endothelial nitric oxide synthase (eNOS), expression of micro-RNAs and lipid metabolism, as well as endothelial dysfunction related to inflammatory cytokines, microparticles and clotting factors. Objective - To evaluate the association of arterial stiffness, eNOS-G894T polymorphism, miRNAs, inflammatory cytokines, microparticles and transfer of lipids with ischemic stroke due to hypertensive emergency. Patients And Methods - The study will be made in patients with ischemic stroke due hypertensive emergency (G1) and will be evaluated three control groups: ischemic stroke without hypertensive emergency (G2), controlled hypertensive and normotensive individuals. The diagnosis of ischemic stroke will be performed by cranium computed tomography. It will be collected peripheral blood samples for analysis of genetic polymorphisms and expression of micro-RNAs by RT-q PCR (quantitative polymerase chain reaction in real time), and inflammatory cytokines, matrix metalloproteinases, clotting factors (ELISA and Multiplex) microparticles (flow cytometry) and lipid transfer (nanoemulsion donor of lipids radiolabelled). The socio-demographic and anthropometric data will be obtained by questionnaire. Arterial stiffness will be evaluated by brachial oscillometric monitor with assessment of central hemodynamics, considering pulse wave speed, central pressure and augmentation index. Patients of both groups will be recruited 3 months after the event for reassessment of arterial stiffness, inflammatory markers and expression of micro-RNAs. For statistical analysis will be used the Kolmogorov-Smirnov, test t or Mann-Whitney test, multiple linear regression and Pearson or Spearman. Significance levels will be accepted for p <0.05. (AU)
